LTC Preauthorized Debit Agreement
Following up to emails sent by the SHA Finance/Accounting office indicating all accounts will be receiving two statements, one for rent & supplies and another for trust charges.
Trust Funds are used for recreation outings, cash withdrawals, charges to but not limited to: canteen cart, hairdressing, special suppers, etc. These charges will be a separate withdrawal from rent & supply statement/charges.
Each resident account must have a Preauthorized Debit Agreement on file for Trust Funds. The Finance/Accounting office suggests a minimum balance of $50. If you do not want any charges for recreation outings, cash withdrawals, charges to but not limited to: canteen cart, hairdressing, special suppers, etc. please enter ‘zero’. Please print and complete the form or stop by the front desk to pick one up (click here for the link to the form). Complete the highlighted sections (Resident Name, tick box Trust/comfort fund line, include an amount, sign & date) and return the form by email to email@example.com or drop it off at the front desk at Providence Place.
