Care Programs
Long-Term Care
We provide care to 159 long-term care residents in single, individual rooms. This includes support with mobility, bathing, dressing, feeding, elimination, and other activities of daily living. Resident care is overseen by Registered Nurses and Licensed Practical Nurses who provide health assessment, wound management, medication administration and management, and many more elements of healthcare. Physicians working in the community attend our facility to provide medical oversight of our residents. In maintaining a holistic approach, the residents are provided with support from a social worker as well as a dietitian
Cognitive Impairment
Guardian Grove is a 29-bed unit dedicated to residents moving along a journey with cognitive impairment. Cognitive impairment can include diagnoses such as Alzheimer’s, dementia, brain injuries, and intellectual disabilities.
On a challenging path, these residents require support with processing their surroundings, peers, and environment in a continuous, personalized way. Located in a secured area of the building, the staff of Guardian Grove is excellent at maximizing the individuals’ memory and capabilities while anticipating physical, emotional, and social needs.
Recreation Team
Our recreation team is comprised of recreation coordinators who work to help our residents achieve social, spiritual, physical, and psychological well-being. The path to achieving this well-being looks different for everyone therefore the recreation team plans and carries out activities aimed to engage the residents as individuals, in small groups, and large group activities.
From trivia quizzes to gardening; pizza nights to sing-a-longs; bocce ball to pub nights; shopping to story-telling; manicures to live music
– our recreation team is always finding new and exciting activities for residents to participate in.
